This mockup (potentially for an upcoming hackathon) begins with a quick, voice-activated entry to make input as simple and immediate as possible. 'Trigger' words and voice-enabled questions can be customized to make entry as intuitive and user-friendly as possible. A manual entry form is also available for less-pressing times or entering multiple seizures.
